Reliant Hospice is committed to providing service excellence to our patients, their families and the referring medical staff and physicians.  Our commitment has earned us the position of first referral source for many of the community physicians.

Attentive & Responsive

We know how anxious your patients and their loved ones can be when you recommend hospice services. Reliant Hospice promises to be attentive and responsive to your patient referrals.  It is our goal to make contact with the patient and their family within an hour of our admissions office receipt of the referral.  We will then assess the referral and admit the patients that are deemed appropriate in a timely manner.

Physicians Participation in Plan of Care

We know that many physicians prefer to participate in the development of their patient’s plan of care. Reliant Hospice promises to keep you as their primary care provider involved in the development of your patients care plan.  We also promise to keep you informed of any changes in your patient’s condition. If you are referring a patient for the first time we will establish communication protocols based on your preferences and requirements.

2. Fax in Referral Form:Simply complete one of our paper referral forms and fax to our admissions office at fax number: 803.798.1188. The referral forms are located in the packet provided by your Relationship Manager or you can print one off from the Reliant Hospice website at the following link:
3. You can call our admissions office and verbally refer the patient.  Our admission phone number is 803.798.1102.
